Achievement Web site Terms of Agreement |
- Eligibility: All nominees must
be a RN or LPN (LPNs may only be nominated in the LPN category).
He/she must hold an unrestricted license, and must practice or
have practiced nursing in Northern Nevada. Candidacy for the category
of Lifetime Achievement is reserved for individuals who have been
a nurse for 25 years or more and who have made contributions to
nursing in Northern Nevada.
- 2012 Nominations will be accepted from Tuesday,
January 1st at 8:00am through Tuesday, January 31st at 8:00am.
- The deadline to accept a nomination is from Monday, February 6, 2012, at 8:00am to Monday, March 12, 2012, at 8:00am.
- The 2012 Award Ceremony is Friday, May 11, 2012, at the Atlantis Casino Resort Spa. Doors open at 5:00 p.m. and dinner is at 6:00 p.m.
- Tickets will be on sale from April 1 - May 6, 2012.
- Upon submitting a nomination, your name will not remain
anonymous unless you select otherwise.
- When accepting a nomination, we strongly suggest that
you complete these questions on a separate electronic document
and then cut and paste your responses into the form fields. This
will ensure your work is safe if the form should malfunction in
any way while you are submitting your response.
- When accepting a nomination, RN/LPN license numbers are required
information.
- By accepting these terms, the nominee gives NOA permission to use their comments to publicize the event
